    DOCUMENT SHREDDING = any papers that contain sensitive, confidential information! The pages MUST be logged on a Records Disposition Log, even if the pages are being sent for shredding.

    "CHEAT" Sheet   Revised 1/27/16
    This sheet gives GENERAL guidelines for the length of time to keep certain records.

    Use this form to identify disposed of (shredded) or stored records
    This form MUST be used if the materials being shredded contain sensitive, confidential information.
    Be sure to include as much detail as possible. [Please don't write "Staff forms' - I need to know EXACTLY what the forms are titled!]
    Please send to Kim O'Connor BEFORE requesting approval from the District Attorney.
    Please send the ORIGINAL to Kim O'Connor - Central Office - 7th Floor.
    Once I receive the original form, I will issue a Work Order for the Warehouse to collect the boxes.
     
    Records Disposition Log    EXAMPLE!!
    This form shows the proper way to complete the Records Disposition Log.
     
     
    Use this form to request for boxes to be picked up by the Warehouse. 
    Enter as much detail as possible if sending boxes to STORAGE.
    The Box ID is unique to your campus or department. [Make up one if necessary!]
    Mark the 'TSLAC Record Number' specific to the materials  being stored. The Record Number is found on the "Cheat" Sheet above.
    Please send the ORIGINAL to Kim O'Connor - Central Office - 7th Floor.

    Label for Storage Boxes     EXAMPLE!!
    Use this label to put on the end of boxes when preparing them for storage.  This is an EXAMPLE only! 
    Simply delete the text in the boxes to type your own info.
