    About Alamo JHS Athletics

    Outline of the Athletic Class

    Each day in athletics students will participate in a variety of strength, conditioning, mobility, and agility activities.  This includes: running, weight lifting, plyometrics, speed ladders, dot drills, floor exercises, and sport specific training. Students who make teams will have additional before or after school sport specific practice.  Students who do not make teams will not be removed from the class, but will also participate in off-season sport specific activities in along with our daily strength and conditioning.  At this age kids develop at varying levels, meaning that just because a team is not made in 7th or 8th that is not the end all be all for their athletic career.

    Expectations for the Athletic Class

    Education comes first.  We want to see each student in athletics become successful, dependable, hard working outstanding graduates.  The athletic class can definitely instill those values into all participating students. First in alignment with UIL guidelines for athletic competition students must make their grades.  Failure to do so will result in ineligibility.  The athletic class also requires students to follow all school rules each and every day.   Students in the athletic class that cannot follow school guidelines will be encouraged to do so until behavior/grades improve or consequently can be removed from the class.  As an athlete you are expected to be a role model for other students and represent Alamo junior high in a positive way.

    Required Student Physicals
           All Alamo JHS student athletes must have a valid MISD Physical Form on file for the current school year before trying out for any sport.  Students must obtain physicals from their own primary care physician.  We advise parents to obtain their child's physical in the summer before school starts. 


    Welcome to the Athletics Parent Portal.



    1. The required annual athletic pre-participation physical and health history cannot be filled out online because the parent/guardian and physician must complete it. You may ask your athletic trainer/coach for a physical and medical history. It is also available on the Midland ISD Athletic webpage. It is also available online at the following link. Once you get the form completed please turn it into your campus athletic trainer or coach.


      2. All other required paperwork will be done electronically. The parent/guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of the primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically.

      3. Remember that to submit online appropriately, you and your child must fill out and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.

    5. Click on for parents tab, then click on fill out electronic forms, then find the state of Texas. You will then scroll down to find MISD. Click on it.


    1. To access the required athletic participation forms for Midland ISD, click the “Online Forms” tab. You will get a list of all required forms. 

      7. To begin, click on the form name and fill out the information requested. Parent/Guardian must have their child present along with the students ID number available to complete the forms. If you do not know the answer to a question, please answer the question with N/A, or none. 

      8. The online forms require both a parent/guardian and student signature. The parent/guardian and student must fill out the forms together 

      9. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create and “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPAD/tablet, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus. 

      10. Once you have filled out all of the information on each page, you will have the opportunity to print the document. 

      11. The hard copy of your physical form and medical history form must be turned in. High School students should turn their hard copies into their campus athletic trainer. Middle School students should turn the hard copy into their coach. 

      12. You will receive a confirmation email once Midland ISD has reviewed the document(s).
