Family & Medical Leave Act Forms (FMLA)

Employee Leave & Job Protection

Family and Medical Leave Act (FMLA)

Midland ISD administers FMLA requests electronically through eFMLA. Employees should use the eFMLA portal to start a request, receive notices, complete forms, upload medical certification, monitor the request, and submit return-to-work documentation.

FMLA at a Glance

Job-protected leave for qualifying needs

FMLA provides eligible employees with unpaid, job-protected leave for qualifying family and medical reasons. Group health coverage continues under the same terms and conditions that would apply if the employee had remained actively at work.

12 months of service generally required
1,250 hours worked in the preceding 12 months
12 Weeks of leave for most qualifying reasons
26 Weeks for qualifying military caregiver leave

What FMLA Provides

Understanding your protections

Job protection

An eligible employee is generally restored to the same or a virtually identical position after returning from approved FMLA leave.

Health plan continuation

Group health benefits continue under the same terms, provided the employee continues paying any required employee premiums.

Continuous or intermittent leave

Qualifying leave may be taken in one continuous period, intermittently, or on a reduced schedule when permitted and supported by certification.

FMLA does not automatically provide additional paid leave.

FMLA is generally unpaid. Available accrued paid leave may run concurrently with FMLA in accordance with District policy. Employees remain responsible for required benefit premiums while on leave.

Electronic Request Process

How to request FMLA through eFMLA

eFMLA is the official electronic system for Midland ISD FMLA requests. Complete each step and continue monitoring your portal and email.

Start the electronic request

Select the red “Start an Electronic FMLA Request” button and enter the requested employee and leave information. Submit the request even when exact dates are not yet known.

Watch for an eFMLA email

eFMLA will send electronic notices and form links to the email associated with the request. Check your inbox, spam, and junk folders. Automated eFMLA email addresses may be unmonitored, so do not reply directly to an automated message.

Create or access your Employee Portal account

First-time users should select Set Your Account Password. Employees who already have an account should sign in. Use Forgot Password before contacting support if you cannot remember your password.

Review your Employee Dashboard

The dashboard is the central location for your cover letter, eligibility notice, rights and responsibilities, certification forms, deadlines, status updates, and other required documents.

Complete and submit required certification

Follow the portal instructions to send the appropriate certification to the health care provider and return it by the stated deadline. Upload requested documents through the secure eFMLA process rather than sending medical information to a general email address.

Monitor notices and report changes

Continue checking the portal and your email. Promptly report changes to the leave dates, frequency, duration, medical status, or expected return-to-work date. Employees must also follow normal District absence-reporting procedures.

Complete return-to-work requirements

When required, submit a Fitness-for-Duty certification before returning to work. This can be completed electronically through eFMLA after an account is established. A paper backup form is also available in the Resources section below.

Quick Answers

Frequently asked questions

Do I submit my FMLA request directly to Midland ISD?

Begin the request electronically using the Midland ISD eFMLA link. eFMLA will provide the required notices, forms, deadlines, and portal access. The Benefits Department remains available for District-specific questions.

What should I do if I have never used eFMLA?

Start the electronic request and watch for an eFMLA email. When prompted, select “Set Your Account Password.” Your account will then provide access to the Employee Dashboard and required forms.

What if I already have an eFMLA account but cannot sign in?

Use the Forgot Password tool with your email address before contacting support. For technical assistance, email support@efmla.com or call (855) 488-FMLA (3652).

Do I still follow normal absence-reporting procedures?

Yes. An eFMLA request does not replace normal District or campus call-in, absence-entry, or attendance-reporting procedures.

Is FMLA paid leave?

FMLA itself is generally unpaid. Available accrued paid leave may run concurrently with FMLA under District policy. Employees must continue paying any required benefit premiums.

Can I use FMLA intermittently?

Intermittent or reduced-schedule leave may be available when medically necessary or otherwise permitted by law. The need and expected frequency should be supported by the appropriate certification.

How do I provide a Fitness-for-Duty certification?

Complete the return-to-work process electronically through eFMLA when instructed. The paper Fitness-for-Duty form is available as a backup. Required clearance should be submitted before returning to work.
