Returning Student Online Enrollment Instructions

Below are instructions for guardians completing the online enrollment for their returning student in Family Access.
- Go to the MISD website and click the Skyward icon.
- Click Login Now.
- Log into Family Access
- Click on the Returning Student Online Enrollment link and select the student for which you need to complete the online enrollment.
Online Enrollment
STEP 1: Verify Student Information
- Verify all information and make changes as needed.
- Click Complete Step 1a and move to Step 1b.
- Repeat steps for 1b – 1e.
STEP 2: Verify Ethnicity/Race
- Read statement and click Continue.
- Verify ethnicity/race information and make changes as needed.
- Click Complete Step 2 and move to Step 3.
STEP 3: Attachments
- Attach documents as needed including any updated documents (i.e. immunization record, proof of residency, etc.). If documents below were previously provided, you do not need to attach again.
- Birth Certificate
- Immunization Records
- Proof of Residency (this must be completed yearly)
- Student’s SSN Card
- Other such as legal documents, custody papers, affidavits, etc.
IMPORTANT: If you are unable to upload attachments, you must bring them to the campus to complete enrollment.
- Click Complete Step 3 and move to Step 4
STEP 4: Student Health Information Card
- Enter all required fields (*), sign, and date form.
- Click Complete Step 4 and move to Step 5.
STEP 5: Authorization for Release of Protected Health Info
- Complete, sign, and date form.
- Click Complete Step 5 and move to Step 6.
STEP 6: Student Residency Questionnaire
- Complete, sign, and date form.
- Click Complete Step 6 and move to Step 7.
STEP 7: Acknowledgement of Electronic Distribution of Student Handbook
- Complete, sign, and date form.
- Click Complete Step 7 and move to Step 8.
STEP 8: Military Connected Student
- Complete, sign, and date form.
- Click Complete Step 8 and move to Step 9.
STEP 9: Medial Release Parental Consent
- Complete, sign, and date form.
- Click Complete Step 9 and move to Step 10.
STEP 10: Network Acceptable Use Policy
- Complete, sign, and date form.
- Click Complete Step 10 and move to Step 11.
STEP 11: School Attendance – A Guide for Parents
- Review form and print if desired. This form is informational only.
- Click Complete Step 11 and move to Step 12.
STEP 12: Migrant Education Program – Family Survey
- Complete, sign, and date form. If answer to question 1 is No, you can proceed to next step.
- Click Complete Step 12 and move to Step 13
STEP 13: Complete Returning Student Online Registration
- Once all previous steps have been completed, click Submit Returning Student Online Registration.
Enrolling Additional Students
Continue registering additional students or click New Student Online Enrollment to enroll students new to the district.
